Retatrutide Ushers in a New Era in Obesity Management

The U.S. Food and Drug Administration (FDA) has approved enlicitide (Lipfendra®), the world's first oral PCSK9 inhibitor, marking a major advance in lipid-lowering therapy. Previously, PCSK9 inhibitors were available only as injectable monoclonal antibodies administered every two to four weeks. Enlicitide is a once-daily oral tablet indicated for adults with primary hypercholesterolemia, including those with heterozygous familial hypercholesterolemia (HeFH), and is intended to be used alongside diet, exercise, and standard lipid-lowering therapy. By inhibiting the PCSK9 pathway, the drug increases hepatic LDL receptor availability, enhancing clearance of LDL cholesterol from the bloodstream.

The approval offers a convenient alternative for patients who require intensive LDL-cholesterol reduction but are reluctant to use injectable therapies or have difficulty maintaining long-term adherence. Oral administration has the potential to improve treatment acceptance and persistence while expanding access to highly effective cholesterol-lowering therapy. As cardiovascular disease remains the leading cause of death worldwide, therapies that simplify long-term lipid management may have a substantial public health impact. Although enlicitide is not expected to be available in India immediately, its approval represents an important milestone in preventive cardiometabolic care and signals a new era in oral precision lipid-lowering therapies.
