A major breakthrough in COVID-19 prevention may have arrived with ensitrelvir, a new oral antiviral developed by the Japanese pharmaceutical company Shionogi. For the first time, an antiviral pill has demonstrated the ability to significantly prevent symptomatic COVID-19 in people exposed to the virus at home. In a large international trial involving more than 2,000 household contacts, individuals who began a five-day course of ensitrelvir within 72 hours of exposure were far less likely to develop COVID-19 symptoms compared with those receiving placebo. Only about 3% of participants taking the drug developed symptomatic infection, compared with nearly 9% in the placebo group. The medication also reduced overall viral transmission, suggesting that it may help limit spread within families and close-contact settings. Researchers describe the findings as an important advancement, particularly for vulnerable individuals such as older adults, transplant recipients, immunocompromised patients, and people with chronic illnesses who continue to face substantial risks from COVID-19 despite widespread vaccination.

Ensitrelvir works by blocking a key viral enzyme required for SARS-CoV-2 replication, targeting the virus during the earliest phase of infection before symptoms fully develop. Unlike some earlier antivirals that showed limited preventive benefit, ensitrelvir achieved clear and statistically significant protection in household exposure settings. The drug was generally well tolerated, with fewer taste-related side effects than those commonly reported with Paxlovid and a safety profile similar to placebo. On the basis of these encouraging results, Japan has already approved ensitrelvir, marketed as Xocova, for post-exposure prevention, while regulators in the United States and Europe are reviewing the therapy. Experts believe that as COVID-19 transitions from a pandemic emergency into a long-term global health challenge, preventive antivirals like ensitrelvir could become an important additional layer of protection alongside vaccination and early treatment, helping reduce severe disease, healthcare burden, and transmission during future outbreaks.
